How it works:
Corticosteroids have multiple actions that produce anti-inflammatory effects, making them widely used for conditions such as asthma. They can stabilize lysosomes in neutrophils, preventing degranulation and reducing the resulting inflammatory response.


Usage And Safety

Dosage:
Betamethasone (as prescribed by a physician)

Side Effects:

  • Common with prolonged high-dose use:

    • Subcapsular cataract

    • Skin thinning

    • Osteoporosis

    • Glaucoma

  • Other possible effects:

    • Features of hypercortisolism (HPA axis suppression)

    • Aseptic osteonecrosis (especially femoral head)

Drug Interactions:

  • Live or other vaccines

  • Anticoagulants

  • Hypoglycemic agents

  • Antihypertensives, diuretics

  • Erythromycin, rifamycin

  • Amphotericin, azole antifungals

  • Ritonavir, cyclosporin

  • Cardiac glycosides

  • Oestrogens (including oral contraceptives)

  • Somatropin

  • Carbamazepine, phenytoin, primidone, barbiturates

  • Muscle relaxants

  • NSAIDs, salicylates

  • Sympathomimetics

  • Mifepristone, methotrexate, theophylline, aprepitant


Indication:
May be required for:

  • Bronchial asthma

  • Severe hypersensitivity reactions (e.g., anaphylaxis)

  • Rheumatoid arthritis

  • Systemic lupus erythematosus

  • Dermatomyositis

  • Mixed connective tissue disease (excluding systemic sclerosis)

  • Polyarteritis nodosa

  • Inflammatory skin disorders (pemphigus vulgaris, bullous pemphigoid, pyoderma gangrenosum)

  • Minimal change nephrotic syndrome

When not to Use:

  • Systemic infections (unless specific anti-infective therapy is given)

  • Live virus immunisation

  • Hypersensitivity to any component


Precautions:

  • History of severe affective disorders (especially steroid psychosis)

  • Previous steroid myopathy

  • Peptic ulceration history

Warnings:

  1. May impair resistance to infection and mask signs of infection (e.g., tuberculosis history).

  2. May suppress pituitary-adrenal axis; insufficiency can persist up to a year after prolonged therapy.

  3. In elderly patients, long-term use increases risk of osteoporosis, diabetes, hypertension, infection susceptibility, and skin thinning.
