How it Works:
Irbesartan is a potent, orally active, selective angiotensin-II receptor (type AT1) antagonist. It blocks all actions of angiotensin-II mediated by the AT1 receptor, regardless of the source of angiotensin-II. This leads to increased plasma renin and angiotensin-II levels, and reduced plasma aldosterone concentration. Serum potassium levels remain generally unaffected at recommended doses. Unlike ACE inhibitors, Irbesartan does not inhibit kininase-II, meaning bradykinin breakdown remains unchanged. No metabolic activation is required for its activity.

Precautions
Patients with bilateral renal artery stenosis or stenosis to a single kidney may be at higher risk of severe hypotension and renal insufficiency when using RAAS blockers. Although not documented specifically with Irbesartan alone, caution is advised.
Warnings
-
Dual blockade of RAAS (ACE inhibitors + ARBs + aliskiren) increases risk of hypotension, hyperkalaemia, and renal impairment. Avoid such combinations.

ACE inhibitors and ARBs should not be used together in patients with diabetic nephropathy.

For patients on diuretics, periodic electrolyte checks are recommended.
