Bosch Qbal Tablets 500 mcg (Pack of 30) – Specification

Generic Name: Mecobalamin (Vitamin B₁₂ analogue) 

Used For:

  • Treatment of peripheral neuropathies (e.g., diabetic neuropathy) in adults

  • Management of megaloblastic anemia due to Vitamin B₁₂ deficiency (in injectable form) 

How It Works:

Mecobalamin acts as a cofactor in nerve function and regeneration, supporting myelin synthesis and repair of nervous tissue. It helps restore normal nerve conduction and supports red blood cell formation. 

Dosage & Administration:

  • Tablet (oral): Typically 3 tablets daily (totaling 1,500 µg/day), divided into 3 doses. Adjust based on patient’s age and clinical response.

  • Injection (IM/IV): 500 µg, 3 times per week for neuropathies or anemia; maintenance dosing may follow after about 2 months.

Contraindications & Precautions:

  • Do not use in patients with hypersensitivity to methylcobalamin

  • Contraindicated in premature neonates and those with hypervitaminosis B₁₂ 

  • Perform intradermal sensitivity test if allergy to cobalamins is suspected

  • Monitor potassium during initial treatment in pernicious anemia cases

  • Treatment efficacy may be reduced by concurrent infection, uremia, folic acid or iron deficiency, or bone marrow-suppressing drugs (e.g., chloramphenicol) 

  • Avoid folic acid supplementation in pernicious anemia as it may mask B₁₂ deficiency and worsen neurological issues 

  • Serum B₁₂ levels may decrease with certain medications (e.g., colchicine, oral contraceptives) or excessive alcohol intake; caution advised

Side Effects:

Anaphylactoid reactions (e.g., hypotension, dyspnea) and hypersensitivity (rash, pruritus) have been reported. Other possible adverse effects include dizziness, headache, injection-site pain, and in rare cases, pulmonary edema or peripheral vascular thrombosis. 

Overdose:

No specific antidote. Management is symptomatic and supportive.
